The Role of EEG in Epilepsy Diagnosis and Monitoring

The Role of EEG in Epilepsy Diagnosis and Monitoring

Epilepsy is diagnosed clinically, from the history of events. EEG gives objective evidence of abnormal brain activity, helps classify the type of epilepsy and guides further tests.

Interictal EEG

Most EEGs are recorded between seizures (interictally). Epileptiform discharges such as spikes, sharp waves and spike-and-wave complexes support the diagnosis and point to a focal or generalised epilepsy.

A single routine EEG has limited sensitivity. A normal result does not rule out epilepsy, and repeat or longer recordings are often needed.

Ways to improve the diagnostic yield

  • Sleep-deprived EEG. Sleep and drowsiness bring out abnormalities. See our sleep-deprived EEG guide.
  • Activation procedures. Hyperventilation and photic stimulation can provoke characteristic patterns, for example in childhood absence epilepsy and photosensitive epilepsies.
  • Prolonged and ambulatory EEG. Hours to days of recording.
  • Video EEG. Captures actual events with synchronised video. See video EEG vs routine EEG.

Ictal EEG and seizure classification

Recording a seizure (ictal EEG) gives the strongest evidence. It shows where seizures start and how they spread, and it separates epileptic from non-epileptic events. This is essential before epilepsy surgery and very useful when diagnosis is uncertain.

Monitoring treatment

EEG can help monitor certain epilepsy syndromes and non-convulsive status epilepticus, and it supports decisions about tapering medication in selected patients, always together with clinical assessment.

What is ictal EEG?

EEG recorded during a seizure; video EEG is the best way to capture it.
