rTMS vs ECT: Differences, Indications and How Clinics Use Both
Both rTMS and ECT treat serious depression, but they work differently and suit different patients. Many psychiatry hospitals offer both.
How they work
- ECT (electroconvulsive therapy) delivers a brief electrical stimulus under general anaesthesia and muscle relaxation to induce a controlled seizure. Modern brief pulse ECT uses much less energy than older devices. See brief pulse ECT explained.
- rTMS uses magnetic pulses to stimulate specific cortical areas while the patient is awake, without a seizure.
Comparison
| Factor | ECT | rTMS |
|---|---|---|
| Anaesthesia | Yes | No |
| Seizure induced | Yes (therapeutic) | No |
| Speed of response | Often rapid | Usually gradual |
| Typical course | Sessions 2–3 times/week | Daily weekday sessions |
| Cognitive side effects | Possible, usually temporary | Not typical |
| Best for | Severe, psychotic, suicidal depression; catatonia | Treatment-resistant depression in outpatients; OCD |
| Equipment cost | ₹84,000 – ₹5 lakh | ₹20 – 24 lakh |
When ECT is preferred
- Severe depression with psychotic features or a high suicide risk.
- Catatonia.
- When rapid response is critical, for example refusal to eat or drink.
- When previous ECT response was good.
In India, the Mental Healthcare Act 2017 requires ECT to be given only with anaesthesia and muscle relaxants.
When rTMS is preferred
- Outpatients with treatment-resistant depression.
- Patients who cannot tolerate medication side effects.
- Patients who prefer to avoid anaesthesia.
- OCD with specific protocols.
