Brief Pulse ECT Explained: Waveforms, Energy and Dosing
Brief pulse ECT is the modern standard. Understanding its parameters helps psychiatrists dose safely and helps buyers compare machines.
From sine wave to brief pulse
Early ECT machines used sine-wave current, which delivered far more energy than needed to induce a seizure. Brief pulse devices deliver short, rectangular pulses that depolarise neurons efficiently. The result is a seizure with less total energy and fewer cognitive side effects.
Key parameters
| Parameter | Typical range (MEDICA BPE-591) |
|---|---|
| Current | 0.8 A constant current |
| Frequency | 20 – 90 Hz |
| Pulse width | 0.1 – 2.0 ms |
| Stimulus duration | 0.1 – 5.9 s |
| Energy | 2.7 – 160 J (at 220 Ω) |
| Charge | 12 – 1400 mC |
Computerised models like the MEDICA BPE-2900 offer 500–900 mA, 20–120 Hz and 0.6–205 J.
Charge: the dose that matters
ECT dose is usually expressed as charge (millicoulombs):
Charge = current × pulse width × frequency × 2 (bi-directional) × duration
Changing the frequency, pulse width or duration changes the charge.
Dosing approaches
- Titration. Find the seizure threshold at the first session, then treat at a multiple of it (the multiple depends on electrode placement).
- Age-based or fixed dosing. Simpler, but less individualised.
Ultra-brief pulse
Pulse widths below about 0.5 ms, often paired with right unilateral placement, can further reduce cognitive effects but may need more sessions.
Safety features to look for
- An impedance check before stimulation.
- Microprocessor energy calculation.
- Auto and manual modes.
- EEG/EMG monitoring to confirm seizure adequacy. See ECT seizure monitoring.

Frequently Asked Questions
What is ultra-brief pulse ECT?
Pulse widths below about 0.5 ms, often used with right unilateral placement to further reduce cognitive effects.
Why constant current?
It delivers a predictable dose regardless of variations in impedance.
What is seizure threshold titration?
Starting low and increasing the dose until an adequate seizure occurs, then treating above that threshold.
